<How to read the table>
In any given area the maximum rank is “area number − 1,” and the condition changes depending on whether your rank is at that maximum or below it.
The upper row is the condition at normal (sub-max) rank; the lower row is the condition at max rank.

On every equipment type and difficulty, reach the AREA 7 Shaft on RULE MODE + Rank EX.
(After you reach it, your rank no longer changes. Once you defeat it a single time, it will begin to appear in STORY MODE as well.)
